Early Head Start: Pregnant women and children from two months to two years and 11 months of age.
Head Start: Children from three years to four years and 11 months of age.
Provide evidence of family income.
Children in foster care, or under the custody of the Department of the Family, are also eligible for our programs.
Documents required to complete the enrollment process for the Head Start and Early Head Start Program:
Proof of age
Proof of most recent residence: Water or electricity bill, lease agreement, or Bona Fide certification
Proof of income: Recent pay stub, W-2, Employer certification, copy of Tax Returns, proof of self-employment, proof of child support (by mutual court agreement or ASUME), Unemployment Denial or unemployment letter, layoff letter, TANF evidence, PAN (SNAP) evidence, Social Security evidence, State Insurance Fund Corporation, Pell Grant, study stipend, and ACCA (all that apply)
Evidence of any health condition or special diet for the child
Evidence of the child receiving early intervention or from the Department of Education diagnosed with an IEP or IFSP (copy)
Evidence of child custody (if under legal custody and not the biological parent)
Proof of studies: Certification or class schedule
Copy of a photo ID of the mother, father, or legal guardian completing the application
